Welcome to the OTHS Nurse
Mission Statement
Students learn best when in an individual optimal state of health. All students may on occasion require assistance at school to achieve or maintain their individual optimal level. A productive learning experience requires a safe and healthy environment. The Nurses at OTHS strive to work with students, staff, parents and community resources to achieve this goal.
INCOMING FRESHMAN:
You will need the following items in order to complete your registration:
- State of Illinois Physical (Sports physicals do NOT count for this requirement) - Due by 10/15 or within 30 days of enrollment
- Immunization Record (Complete and up to date) - Due by 10/15 or within 30 days of enrollment
- Dental Examination Record - Due by May 15th
ALL STUDENTS:
- Tylenol/acetaminophen requires a completed consent form, which is good for all years at OTHS.
- All other medications require yearly (school year) consent.
If you have rescue medications (Epi-pen, inhaler, etc.) an Action Plan is required annually (school year). A Diabetic Medical Management Plan is required for diabetes.

NEW STUDENTS:
You will need the following items in order to complete your registration:
- Illinois Eye Exam or Eye Exam Waiver
- State of Illinois Physical (Sports physicals do NOT count for this requirement)
ALL SENIORS:
You will need the following items in order to complete your registration:
- 2nd of the Meningitix Vaccine (MCV)
*COVID vaccinations are not required, but encourage those vaccinated to submit their vaccination record.
