Illinois Eye Examination Requirement

Required for out of state students
 

Any student, regardless of age, moving from out of state into District 203 must show proof of having an eye examination by a physician licensed to practice medicine or a licensed optometrist within the previous year. This examination must include history, visual acuity, best corrected and uncorrected vision, internal and external examination, and glaucoma evaluation. A waiver is allowed for students who show undue burden or lack of access to a physician licensed to do eye examinations. The Illinois eye examination form and waiver are available in the form section.
